Required Forms for Summer Resident Camp

This year, ALL adults attending Summer Resident Camp at Camp Muhacke must submit a Central Registry Clearance Request form (linked below) to the following address:Department of Human Services

Mark yourself as the Individual requestor of the clearance, then bring the form you receive in return to camp. This form should be submitted ASAP to ensure time for processing!

Also, as in past years, all youth and scouts must complete a health form and bring it to camp. That form can be downloaded here..
